Come and care with us

We focus on relationships, not rota slots. Tell us about yourself and we will be in touch within 10 working days.

About 10 minutes to complete · progress saves automatically on this device · Amani Care Ltd is an equal-opportunity employer committed to fair treatment and diversity.

Before you start, please have ready:

  • Your contact details
  • Your work history for the last few years
  • Whether you have the Right to Work in the UK
  • Whether you have a DBS check (if you have one)

1. Personal details

We use this to match you with rounds close to home.

2. Right to work in the UK

Important information

All applicants must have the legal right to work in the UK. We do not ask for any documents or ID numbers at this stage — no passport, share code or National Insurance number. If we invite you to interview, we will talk you through the right-to-work check then.

Are you eligible to work in the UK? *

3. Emergency contact (next of kin)

Optional at this stage — you only need to give us an emergency contact if we offer you work. Feel free to leave this blank for now.

4. Availability & travel

Please tick all days and times you are available to work. You may select multiple options across different shifts.

Getting between visits

Most of our rounds involve travelling between homes across the borough. Not driving is absolutely fine — we simply use this to plan rounds that work for you.

Do you hold a full UK (or exchangeable) driving licence? *
Do you have use of a car for work? *

5. Employment conduct

Do you have restrictions on your working hours? *
Have you ever been dismissed from any employment? *
Have you ever been, or are you currently, subject to any investigation or disciplinary action? *

6. Interview adjustments & vetting

Do you need any adjustments to take part in an interview or assessment? (optional)

We ask only so we can make your interview accessible — for example a ground-floor room, extra time, an interpreter, or the questions in advance. We do not ask about your health or disability when deciding who to interview. If we offer you a role, we will then ask in confidence about anything that affects the work and any adjustments that would help.

Are you subscribed to the DBS Update Service? *

7. Full employment history

List everything since you left secondary school — every job (paid, voluntary, agency or self-employed) and any college, university or other study. We'll check your timeline as you go and point out any gaps for you, so there's nothing to work out yourself. No need to cross-check your CV.

The month and year you finished school (usually age 16–18) is enough. Add any college or university after that as a Study entry below.

Your jobs and studies

Start with your current or most recent job or course and work backwards. Choose Study for college, university or other courses.

8. Gaps in employment

Any break of one month or more shows up here automatically — just tell us what you were doing. A few words is fine. Gaps are completely normal; we ask because safer-recruitment rules for care work require it.

Add when you left school and your jobs and studies above — any gaps will appear here.

9. Education & qualifications

Please list all relevant qualifications, including Care Certificate, NVQ, GCSEs, A-Levels, degrees and professional certifications.

10. Personal statement

Three short questions — no essay needed

A few sentences for each is plenty. Honest answers matter more than long ones.

What care roles have you done, paid or unpaid? What kinds of clients have you supported? A few sentences is fine.

0 / 1500 characters

What drew you to us specifically? One or two sentences is enough.

0 / 1000 characters

A real example — a client, family member, or anyone you've cared for. What did you do and why?

0 / 1500 characters

11. Professional references

Please provide two professional references. One must be your current or most recent employer. References should be from supervisors who can speak to your work performance and character.

We will not contact your current employer without your permission. References are only taken up if we make you a conditional offer, and we will always let you know first.

12. Declaration & convictions

Legal declaration — please read carefully

This role is exempt from the Rehabilitation of Offenders Act 1974, so you must declare any convictions, cautions, reprimands or final warnings that are not "protected" under the DBS filtering rules — including some that are otherwise spent.

A disclosure is not an automatic bar. We consider each case individually, looking at how long ago it was, how relevant it is to the role and what has changed since. You do not need to give any details here — if there is anything to discuss, we will do so in confidence with you, and only if your application progresses. Deliberately withholding a disclosure may result in withdrawal of any offer.

Do you have any convictions, cautions, reprimands or final warnings that are not "protected" under the DBS filtering rules? *

No details needed on this form.

Are you barred from working with adults or children, or subject to any sanction, prohibition or ongoing investigation by a professional or regulatory body? *

For example the DBS barred lists, the Nursing and Midwifery Council, Social Work England or a similar body.

Equal opportunities monitoring — optional, opens on click

Amani Care is an equal-opportunity employer. These questions help us check that our recruitment is fair to everyone. They are entirely voluntary, are recorded anonymously and separately from your application, and are not seen by the people deciding who to interview. You may answer none, some or all of them.

How we use your information. Amani Care Ltd is the data controller for the details you give us here. We use them to assess your application and, if you are successful, to meet our legal and CQC recruitment duties — our lawful bases are our legitimate interest in recruiting safely and our legal obligation as a regulated care provider. Your information is stored securely and seen only by the people involved in recruitment.

  • How long we keep it. If your application is unsuccessful we delete it 6 months after we tell you our decision. If you join us, it becomes part of your employment record and is kept as our staff-records policy sets out.
  • Your saved progress. So you can finish later, this form saves what you type in your own browser on this device — not on our servers — until you submit it. Use "Start fresh" at the top to erase it, or clear your browser data.
  • Your referees. Please make sure the people you name are happy to be contacted. We will not approach any referee — and never your current employer — until we have made you a conditional offer and told you first.
  • Monitoring answers. Any equal-opportunities answers you choose to give are recorded separately and anonymously, are not linked to your name or application, and are never seen by the people deciding who to interview.

You can ask us for a copy of your data, or ask us to correct or delete it, at any time — email info@amanicare.co.uk. See our Privacy Policy for full details, including your right to complain to the ICO.
